How does vertex distance affect spectacle to contact lens power?

When moving a lens closer to the eye (reducing vertex distance from 12mm to 0mm for contact lenses), a minus (-) lens becomes effectively stronger and requires less power, while a plus (+) lens becomes effectively weaker and requires more power. Vertex compensation is calculated using the formula: F_new = F / (1 - d * F).

Clinical Principles of Back Vertex Distance (BVD)

Back Vertex Distance (BVD) is defined as the distance between the back surface of a spectacle lens and the anterior surface of the cornea. In clinical optometry and ophthalmic dispensing, vertex distance plays a critical role whenever refracting or fitting prescriptions exceeding ±4.00 Diopters.

As a lens is moved closer to the corneal apex:

  • Minus Lenses (-D): Gain effective minus power. Therefore, when prescribing contact lenses (0mm vertex), the prescription must be reduced in minus power.
  • Plus Lenses (+D): Lose effective plus power. Therefore, when prescribing contact lenses, the plus power must be increased.
